
How to Prepare for Your Anesthesia Appointment
Selkirk Dental Anesthesia PLLC provides mobile anesthesia services directly inside your dentist’s office, offering a safe, seamless, and stress-free experience for patients. The steps below outline exactly how to schedule anesthesia for your upcoming dental procedure—from completing your forms to receiving clearance, finalizing your appointment, and preparing for the day of treatment. Our goal is to make the process simple, transparent, and supportive at every stage.
Complete Required Forms
Complete the health history questionnaire and financial agreement through IntakeQ, our HIPAA-compliant platform. If additional information is needed (labs, testing, or medical records), it is the responsibility of the patient and/or referring office to upload these documents directly through IntakeQ.
The dental office will complete a Financial Agreement form with you that will give you an estimate for the total anesthesia fees due.
Schedule Appointment
After submitting all medical history we will determine if you are a good candidate for dental anesthesia. Once you receive clearance you will work with dental office manager/coordinator to select your appointment date and time.
Pre-Procedure
You will receive a phone call from your anesthesia provider 1–3 weeks before your scheduled date.
You must complete this call prior to your procedure day to avoid potential cancellation.
****Full payment is due no later than the day before your procedure.
The initial amount due is based on an estimated procedure length provided by your dentist, and final charges are subject to change based on the actual duration of the procedure. If you are the only case scheduled at an office for a day a minimum charge of 2-hours of anesthesia time will be billed. Additional time will be billed in 15-minute increments. If you are covered by state insurance you may not owe anything for anesthesia fees, this will be confirmed by our billing specialist prior to the procedure.
