STEPS FOR PAYMENT FOR DENTAL SEDATION

  1. Dental office will assist you in booking sedation appointment. All intake forms MUST be completed and clearance for anesthesia granted by Selkirk Anesthesia at time of booking.

  2. At time of booking a $500 non-refundable deposit is required.

  3. Day of sedation appointment full payment based on time estimate from dentist is required before sedation will begin.

  4. If estimated time is longer a balance will be billed to patient due that day. If time is less than estimated a prorated refund will be given to the patient that day.
